Loose circuit card behind ICBM incident at Warren

Last year, 50 ICBMs at F.E. Warren Air Force Base were affected by a communications glitch for nearly an hour. Investigators determined that a loose circuit card was the cause.

The card, inside a weapons-system processor, had not been properly locked after maintenance work had been done. More than nine hours later, the card came loose and the weapons-system processor started to sent out garble communications to 50 missiles.
