USAF C-130J crash investigation determined that pilot had jammed yoke with NVG case

A USAF C-130J, #08-3174, crashed in Afghanistan on Oct. 2, 2015 killing 11 people on board and three Afghan soldiers on the ground. An investigation by Air Mobility Command concluded that the mishap pilot had place a hard shell night vision goggle (NVG) case in front of the yoke during offloading of cargo and forgot to remove it during take-off.

The cargo plane pitched up rapidly during take-off and the crew was not able to recover the plane from a subsequent stall.

The aircraft impacted approximately 28 seconds after liftoff and hit a tower with three Afghan Special Reaction Force (ASRF) members on it.
