Investigation report on crash of BAMS-D faults pilot

The U.S. Navy determined that the crash of a Broad Area Maritime Surveillance-Demonstrator (BAMS-D) was due to a faulty ruddervator.

However, the pilot at the controls also violated emergency procedures by not commanding the aircraft to climb to above 40,000 feet.
