Royal Canadian Air Force evaluators warned nine years ago that the Sikorsky CH-148 Cyclone helicopter could have issues affecting its performance.
Despite the warning, Paul Martin-led government went ahead and awarded Sikorsky the contract to convert the S-92 into a naval helicopter.
Engineers had found issues with engine performance, acoustic noise and electronic interference in the cabin.
