There were nineteen times Red Arrows crew could have spotted a safety risk of the ejection seat on a Hawk T1 before it killed Flt Lt Sean Cunningham.
The inquest into his death was told that the seat handle was left in a stage just one step from firing at least four days leading up to the accident.
