Minuteman accident was not reported to Pentagon review team

Back in 2014, former U.S. Secretary of Defense Chuck Hagel commissioned a review of the entire U.S. nuclear force by a group of nuclear weapons experts. The review started in March and it was during that period when an accident at F.E. Warren Air Force Base resulted in $1.8 million in damage to a Minuteman missile.
