USAF releases investigation report into T-38 crash that killed Japanese student pilot

The U.S. Air Force has determined that human error was the cause behind the fatal crash of a T-38C in February this year. The crash killed the student who is from Japan and his American instructor.

Three Egyptian air force F-16 Fighting Falcons fly in formation during Bright Star 21, over northern Egypt, Sept. 16, 2021. The 93rd EARS supported aerial refueling during Bright Star to both ensure participation from U.S. Air Force F-15E Strike Eagle aircraft assigned to the 332nd Air Expeditionary Wing and provide training qualification opportunities for 12 Egyptian air force F-16 Fighting Falcon pilots. Bright Star 21 occurred from Sept. 2-16, 2021, and comprised three main events: a command post exercise, field training exercise and senior leader seminar.
