A Chinook helicopter crash in Afghanistan in August that killed 38 U.S. and Afghan troops was not due to any mistakes.
The aircraft was hit by a rocket-propelled grenade on the rear rotor and one of the rotor blades, resulting in a catastrophic explosion.
