Petty Officer 1st Class Regan Young was killed on board USS Essex while the ship was off the coast of Bali, Indonesia last year after getting trapped by the ship’s Sea Sparrow missile launcher.
Investigators said Young might have ignored warning signs and sat underneath the launcher. Sailors maintaining the missile launcher also failed to check the area around the weapon before carrying out work.
