The U.S. Navy has relieved the duties of the commanding officer and command master chief of HM-15 due to loss of confidence in their abilities to command.
Cmdr. Sara Santoski was found to have failed to strictly enforce appropriate operational, maintenance and safety standards which lead to a crash in July and another mishap in August.
