Pilot error lead to Pave Hawk crash in Okinawa

The pilot of a USAF HH-60G Pave Hawk was maneuvering at low-altitude when the helicopter crashed in Okinawa last year.

The investigation board found that the pilot had misjudged and thought the aircraft was on a collision course with another helicopter.

His subsequent actions plugged the aircraft into the jungle, resulting in the total lost of the aircraft and killing a crew member.
