USS Bonhomme Richard could have been saved if not for sweeping failures

The U.S. Navy has identified 36 individuals as having contributed to the loss of USS Bonhomme Richard.


Five of them are admirals while nine are civilians.

The maintenance reports were falsified, and 87% of fire stations on board were not inspected or had equipment problems, the investigators found.

For full ten minutes after spotting the fire, sailors failed to alert others and did not activate the firefighting foam system.

“No member of the crew interviewed considered this action or had specific knowledge as to the location of the button or its function,” the report said.
