Maintenance error, wrong decision by crew lead to fatal UH-1Y crash
On Jan. 23, a UH-1Y crashed near Marine Corps Air Ground Combat Center Twentynine Palms, killing both crew members on board. An investigation into the accident found that the fatal accident could be avoided.

F/A-18C from VMFA-232 crashed in U.K. while returning home from Middle East
A F/A-18C from VMFA-232 crashed near Royal Air Force Lakenheath, England while returning to its home base at MCAS Miramar on Oct. 21. The pilot was killed in the crash.

F-35 ejection seat issue expected to be resolved by 2017
F-35 Joint Program Office says three fixes are available to solve the recent ejection seat issue. All three solutions are expected to be implemented by summer 2017, the office has said.
