Former chairman of the Joint Chiefs of Staff Hugh Shelton says in his new book that former U.S. President Bill Clinton had lost a card containing codes need to launch a nuclear attack.
The Pentagon was kept in the dark about the missing card until it was time to change to a new set of codes.
