Engine blade detachment cause of RCAF CT-155 Hawk crash in 2008

The crash of a RCAF CT155 Hawk at 15 Wing Moose Jaw in 2008 was cause by a catastrophic engine failure after a blade broke off.

Investigators found that the crash could be linked to another crash last year and now all engine blades would have to be replaced every 400 hours.
