The U.S. Air Force has determined that human error was the cause behind the fatal crash of a T-38C in February this year. The crash killed the student who is from Japan and his American instructor.

Investigators found that the student had left the throttles at idle for 18 seconds and by the time the instructor called for an abort for landing, the aircraft had lost too much speed and impacted the ground.
